To serve you to the fullest, we need your approval to conduct our sessions. This form simply acknowledges your consent and explains the limits of our liability.
Authorization Form
In some circumstances, you may want us to communicate with other professionals on your care team (e.g. doctor, psychiatrist, etc). This form simply acknowledges your consent.
